Explanation of Study and Informed Consent Form:
Capital
Area Community Information (CACI)
The purpose
of this study is to: 1) understand the usability of the CACVoices Website, 2)
to recommend changes to the Website based on what the study reveals, 3) work
with participants to help them use CACVoices and the information it provides to
achieve individual, community, and organizational goals (and to provide data
and information for CACVoices if desired). The participation we are asking for
here consists of filling out a small online survey.
The results of this study will be used in research presentations, journal/magazine articles, and in book chapters. Results will also be used to improve CACVoices and help participants become more effective users of the Website. Your name and the name of your school will be changed in any published studies to protect your privacy.
Your privacy will be protected to the maximum extent allowable by law. I, the researcher, will have access to study data, as will any student attached to the project as a research assistant. All electronic data will be saved on a password-protected computer and on a password protected server. All paper data and other media, such as audio tapes, video tapes, and computer disks will be kept in a locked file cabinet. All data will be kept for a minimum of 3 years. There are no anticipated risks associated with your participation in this study. Your participation is voluntary; if you refuse to participate there will be no penalty, and you may discontinue participation at any time.
